Safety implications identified included lack of spatial allowances, disjointed organization of work, lack of technical know-how, system malfunctions, and interruptive work processes that affected key work activities. Since each activity in the dispensing journey is linked to the next and has a direct effect on all other dispensing activities, a failure in the journey snowballed into other failures.
